Treating HIV and preventing it at the same time

Treatment drugs can do more than improve the health of people with HIV: If administered early, medications can also reduce the spread of the disease to sexual partners and may help stem the AIDS epidemic. But many logistical hurdles stand in the way of making this strategy feasible, affordable and effective, according to experts writing in Tuesday's edition of the journal PLoS Medicine.

The medications in question are antiretroviral therapies, which prevent HIV from multiplying and drastically diminish the amount of virus circulating in the blood. This not only curbs HIV's assault on the immune system, it also makes it much more difficult for an infected person to pass the virus to someone else.

This dual benefit was recognized last year in a landmark study finding that people with HIV who received treatment early on, before their immune systems encountered much damage, were 96% less likely to transmit the virus to their heterosexual partners than those who received treatment later in the course of the disease, when drug treatment is usually initiated.

One important implication was that HIV treatment and prevention efforts didn't have to compete for resources, said Dr. Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, part of the National Institutes of Health. The study prompted AIDS experts to rethink their approach to preventing the virus' spread. Science magazine named it the "breakthrough of the year."

But capitalizing on this discovery may prove more difficult than some had hoped. Experts are now divided about whether the treatment-as-prevention approach can essentially halt the AIDS epidemic.

"The field is split about whether it's really the best thing and it's going to stop transmission, or if it's a small part of the puzzle," said Timothy Hallett, an infectious disease epidemiologist at Imperial College London, who co-wrote one of the new studies.

The real-world experience of public health officials in San Francisco, British Columbia, France and Australia ? four places where HIV testing and drug therapy are already common ? make clear that simply handing out pills won't be enough to beat back the virus, according to one of the studies.

Australia probably represents a best-case scenario, since antiretroviral drugs are freely available in that country and about 70% of people with HIV take the medication. But even there, the number of new HIV diagnoses has increased from 700 per year in 1999 to 1,000 per year in 2011, said study author David Wilson, an epidemiologist at the University of New South Wales in Sydney.

Early treatment "can have an impact, but it's not going to eradicate HIV," he said. "It's not the magic bullet."

For treatment to double as prevention, people need to get tested for HIV on a regular basis, start antiretroviral treatment once they test positive, and swallow pills for the rest of their lives to manage their viral load. Failure to follow through at any of these stages greatly reduces the drugs' ability to block transmission, Wilson said.

Making matters worse, people on the medication may stop taking other preventive measures, like using condoms, he added.

Biology and behavior are only two aspects of the treatment-as-prevention strategy that were examined in the PLoS Medicine studies. Some of the papers addressed economic considerations, including how long it would take for expenditures on antiretroviral drugs to pay for themselves by preventing costly treatments in people who remain HIV-free. The answer to that question is important for policymakers who are working with limited budgets.

Study authors also suggested that public health officials prioritize getting drugs to sex workers (who could spread the virus widely if untreated) and pregnant women (to reduce transmission to babies and deaths of mothers), among other groups. However, the authors noted the paucity of data available to evaluate whether these are the right groups to target.

Fauci, who didn't work on the new studies, said a primary objective was reaching and treating people who didn't know they were infected with HIV, since they unwittingly transmitted the majority of new HIV cases each year.

Public health officials in Los Angeles County, which has the country's second-largest concentration of HIV-positive people, hope to make testing a routine part of healthcare services to reach such individuals. Mario Perez, director of the county's HIV and sexually transmitted disease programs, called early treatment a "cornerstone" of the HIV prevention program.

"Getting a much higher proportion of residents living with HIV to be on treatment is going to be crucial to getting ahead of the epidemic," he said. "If we don't do that, I think we're going to continue to limp along with trying to prevent HIV infection."

And while the costs of drug treatment are much lower than they used to be, thanks in part to the development of generic alternatives to expensive name-brand drugs, other prevention methods like circumcision and condom distribution are far less costly.

Experts emphasized that early drug treatment should not be seen as a replacement for other preventive measures.

Though the strategy faces many uncertainties, Fauci said one thing was clear: "Biologically, it absolutely works." And if it can be successfully implemented, he said, "it would be an absolutely critical part of turning around the trajectory of the AIDS pandemic, in this country and worldwide."

Medical report: Zimmerman nose 'likely broken'

The Florida block watch captain who shot and killed Travyon Martin awaits a judge's ruling on his new bond application. NBC's Jeff Rossen speaks with Mark O'Mara, George Zimmerman's attorney.

By NBC's Jamie Novogrod

SANFORD, Fla. -- The day after the encounter that left 17-year-old Trayvon Martin dead, George Zimmerman visited a doctor?s office in nearby Altamonte Springs, where he was told his nose was ?likely broken,? but the wounds at the back of his head did not require stitches.

Those details ? along with other notes on Zimmerman?s injuries and his health in general ? are revealed in a medical report prepared by a physician?s assistant after a Feb. 27 visit.

The document was among a rash of evidence filed in Seminole County Court on Friday and released to the public Tuesday by Zimmerman?s attorney, Mark O?Mara.

The document, which NBC News first reported on May 16, offers some insight to Zimmerman?s injuries ? and, perhaps, his state of mind in the aftermath of his encounter with Martin.

NBC News

George Zimmerman appears in court in Sanford, Florida, on Friday.

Zimmerman had no slurred speech or dizziness as a result of his injuries, the report notes, but adds that Zimmerman admitted to ?occasional nausea when thinking about the violence last night.?

According to the report, it was recommended Zimmerman be evaluated by an ear, nose, and throat specialist, but he refused.? The report also says that Zimmerman had been told he needed medical clearance to return to work.

Zimmerman, 28, shot Trayvon Martin dead during an altercation on Feb. 26th inside the gated community where Zimmerman was a neighborhood watch coordinator.

Zimmerman, who has pleaded not guilty to second-degree murder charges, told? Sanford police he acted in self-defense after Martin punched him and repeatedly banged his head against the pavement.

In court Friday, defense attorney Mark O?Mara pointed to Zimmerman?s wounds as evidence supporting a self-defense claim.

The report released Tuesday makes note of two lacerations at the back of Zimmerman?s head ? one 2 centimeters, and the other a half-centimeter ? but adds that they don?t require stitches.

The report also says Zimmerman?s nose was ?likely broken,? and that he complained of pain to his lower pelvis.?

Zimmerman was given a prescription for the painkiller Lodine, according to the report.? It also shows that Zimmerman complained of ?stress,? and had been prescribed an anti-anxiety drug, Temazepam, in January, and a drug for hyperactivity, Adderall, earlier in February.

Also Tuesday, O?Mara released freeze frames of Trayvon Martin from a video captured by security cameras at 7-Eleven the night he was shot.?

As is widely known, Martin visited the store to buy Skittles and an Arizona brand drink before walking to this father?s girlfriend?s house inside the Sanford gated community.

Reached by telephone Tuesday, O?Mara said he intended to ensure the public understood how Trayvon looked on the night of his death.

"These are the most recent pictures of the Trayvon Martin that intersected with George Zimmerman that night," O'Mara told NBC News.

O?Mara added that the pictures show that Trayvon was taller than the store clerk, who O?Mara said is closer to Zimmerman's height.

Seminole County circuit Judge Kenneth Lester is expected rule Thursday on Zimmerman?s bond after a review of the evidence submitted by O?Mara.

Lester last month revoked Zimmerman's $150,000 bond after prosecutors said Zimmerman and his wife, Shellie, had misled them about his finances at an April bond hearing.

Happy 50th, soon-to-be-single Tom Cruise

Paramount Pictures / Getty Images

Tom Cruise couldn't have asked for a more dramatic 50th birthday week. On Friday, the world learned that Cruise and Katie Holmes were splitting up, leading to all kinds of?buzz about the age of Cruise's three wives when their marriages broke up, Holmes' acting future, and even?Rupert Murdoch's thoughts on Scientology.

It's hard to separate headlines relating to Cruise's personal life from his movies. But start recalling his roles, and you realize he's starred in some classics over the years. "Taps." "Risky Business." "A Few Good Men." "Top Gun." And like any good actor, he's capable of reinventing himself -- witness "Tropic Thunder."

So happy half-century mark, Mr. Soon-to-be-Single Cruise. In your honor, here are some of our staff's favorite scenes from your movies.

?Risky Business?
The 1983 movie that launched a young Tom Cruise to superstardom still holds up as a quintessential coming-of-age tale. Who hasn?t attempted to imitate the actor?s lip-syncing slide across that wood floor in?just socks and underwear? But the scene that sticks in my mind is high-schooler Joel Goodson?s talk with a Princeton recruiter in the midst of an epic house party. With call girls and horny teens interrupting, and the prospect of an Ivy League education apparently slipping away, Cruise?s character delivers a line straight out of an Internet age that?s still years away. ?If there?s one thing I?ve learned in all my years, sometimes you gotta say, ?What the f---??? With his signature Wayfarer sunglasses and ear-to-ear grin, Cruise created an iconic movie-star moment that left just one response for anyone who would question his future success: WTF????? --Kurt Schlosser

?'A Few Good Men'
Everyone remembers Jack Nicholson's "you can't handle the truth!" scene from this 1992 movie. Look beyond Nicholson's awesome performance?and you'll?notice Cruise standing right?in there with him as young military lawyer Daniel Kaffee, going toe to toe with Nicholson's imposing Col. Nathan Jessep. Cruise's voice?trips up and down the scale,?gently purring?one second, then rising to a bellow the next. In a true Perry Mason minute, he's about to get Jessep to confess on the stand,?and right up until it happens you believe that his character doesn't exactly know what he's stumbled upon. But when?Cruise rises to a crescendo, you can hear the punctuation in his voice. COLONEL JESSEP! DID YOU! ORDER! THE CODE RED? You're goddamn right he did.??--Gael Fashingbauer Cooper?

?'Taps'
When ?Taps? came out in 1981, Tom Cruise was?far from famous. The actor had just one other professional film credit ?- as a budding arsonist in ?Endless Love? ?- and in ?Taps? he seemed well on his way to crystallizing a career as?an actor who only played psychopaths.?As the rest of his fellow cadets throw down their weapons and turn over their beloved school to the authorities, Cruise -- firm-jawed, sporting a red beret and a slight hint of madness in his eyes -- ?sprays the troops with gunfire; when his fellow school chums Sean Penn (in his first feature role) and Timothy Hutton race to stop him, he turns and shouts, ?Isn?t it beautiful?!? The result? Hutton dies in Penn?s arms, giving Tom Cruise one more feather for his cap: He can take credit for having made Sean Penn cry in a movie.??? --Randee Dawn

?Top Gun?
?Top Gun? was Tom Cruise at his best: handsome, high-energy, charismatic. His portrayal of the cocky fighter pilot, Maverick, in the 1986 film firmly established Cruise as a Hollywood superstar.? In a classic scene, he tries to impress the smart and sexy Charlie, played by Kelly McGillis. His brash, off-key serenade to The Righteous Brothers? ?You?ve Lost That Loving Feeling? earns him a begrudging invitation to sit?down next to her?? and sparked karaoke imitations everywhere.??? --Joy Jernigan

?Tropic Thunder?
Cruise is known for playing the smooth lead in blockbusters. So when he unexpectedly appeared as fat, hairy, balding, foul-mouthed studio head Les Grossman in Ben Stiller?s 2008 comedy ?Tropic Thunder,? jaws dropped -- and moviegoers couldn?t stop laughing. The confidence that Cruise always brings to his roles was there in ?Tropic Thunder,? but with Les, he delivered a new level of comedy that fans hadn?t really seen before. To be more specific, he showed off some amazingly awesome and slick, booty-shaking (and oddly sexy) dance moves to the tune of Flo Rida?s ?Low.? (You can watch his full, 15-minute dance here, but beware of explicit lyrics.) The small role was such a hit, Cruise brought Les back at the 2010 MTV Movie Awards to give his young ?Risky Business? self some advice on set and shake his moneymaker with Jennifer Lopez. Rumor has it that Les is even getting his own full-length feature film.??? --Anna Chan

Conduct code for unmanned aircraft is unveiled

(AP) ? A trade group for drone aircraft manufacturers and operators has released the industry's first code of conduct in response to growing privacy concerns.

The Association for Unmanned Vehicle Systems International said Monday that the recommendations for "safe, non-intrusive operation" are meant to guide operators and reassure a public leery of the possibility of spy drones flying undetected over their homes.

"We understand as an industry that we've got a public relations problem," said Paul McDuffee, a director of the association who helped draft the recommendations.

Drones, small airplanes or helicopters operated remotely by pilots from the ground, can be equipped with sophisticated cameras and even weapons. They have been used to spy on and hunt down al-Qaida terrorists in Pakistan, but the rapidly declining size and cost of them has prompted fears that thousands could be operating in the U.S. within a decade, with little effective oversight. Some of the drones weigh just a few pounds and can fit in a person's hands.

Citizens, civil liberties groups and politicians have voiced worries that the small aircraft raise the specter of a "surveillance society." Currently there are only about 300 authorized federal permits to operate such aircraft, along with an unknown number of unlicensed amateurs, who are supposed to keep their aircraft within sight.

The new recommendations by the association, a non-profit based in Arlington, Va., that has members in more than 60 countries, pledge to "respect the privacy of individuals" and the concerns of the public and to follow all federal, state and local laws. They also pledge to ensure that remote drone pilots are properly trained and to respect "other users of the airspace."

The language on privacy is good, but it's not enough, American Civil Liberties Union lobbyist Chris Calabrese said.

"I think it's really important that they're paying attention to privacy. That's to their credit," Calabrese said. "But I can't imagine they expect this to quell privacy concerns."

Calabrese added that ultimately even well-meaning guidelines from a private group aren't legally binding on public and private organizations around the country.

"I think Congress needs to step in. This is new technology. It's potentially incredibly invasive," he said. "People are profoundly discomforted by the idea of drones monitoring them."

Some law enforcement agencies have already purchased powerful drones.

A county sheriff's office in Texas used a homeland security grant to buy a $300,000, 50-pound ShadowHawk helicopter drone for its SWAT team. Randy McDaniel, chief deputy with the Montgomery County Sheriff's Office, said earlier this year his office had no plans to arm the drone, but he left open the possibility the agency might decide to adapt it to fire tear gas canisters and rubber bullets.

McDuffee, who works for Insitu, a Boeing Co. subsidiary that designs and builds drones, called the wide range of drone aircraft "the next latest and greatest thing in aviation," noting that there's interest from law enforcement, first responders, scientists and private industry, such as farmers who want to monitor or spray crops. The permits are issued by the Federal Aviation Administration, which is working on new rules that will greatly expand legal uses.

The drone market is expected to nearly double over the next 10 years, from current worldwide expenditures of nearly $6 billion annually to more than $11 billion, with police departments accounting for a significant part of that growth.

Earlier this year, Congress, under pressure from the U.S. Department of Defense and drone manufacturers, ordered the FAA to give drones greater access to civilian airspace by 2015. The mandate, besides applying to military drones, applies to drones operated by private companies and government agencies, including federal, state and local law enforcement.

But privacy concerns have already prompted members of Congress to introduce bills to prohibit any government agency from using a drone to "gather evidence or other information pertaining to criminal conduct or conduct in violation of a regulation" without a warrant.

McDuffee said there are many uses for drones that wouldn't generate controversy. For example, scientists have used drones to monitor airborne pollution levels and numbers of sea lions in the Arctic and to monitor hurricanes.

"They go in places where you would never consider risking a human life," he said.

He added that drones also could be used to create temporary communications networks after natural disasters, if regular service went out.

"They're almost like miniature satellites," McDuffee said. "That's the beauty of these systems."
